Must be on a point to point basis (Hotel to Center/Hospital and v.v.) 4. Individuals who will take their NCLEX Exams for Nurses In order for us to confirm your booking, we would like to know the purpose of your stay. Please note that at the moment, we are not yet accepting staycation and leisure purposes. LIST OF REQUIREMENTS THAT YOU NEED TO PRESENT PRIOR/UPON CHECK-IN: If taking NCLEX or other Medical-related Appointments (scheduled RT-PCR and/or ANTIGEN for Medical reasons) • Valid ID • Health Declaration Form (to be accomplished upon check-in) • Proof of Appointment / Examination Schedule Confirmation / Schedule of Medical, RT-PCR and/or ANTIGEN • Swab test result before check-out (for Medical purposes) If a Health Workers who need easy access to place of work: • Valid Company ID • Health Declaration Form (to be accomplished upon check-in) • Proof of work schedule from company If Individuals who are required to undergo mandatory Quarantine: • Valid ID • Health Declaration Form (to be accomplished upon check-in) • Proof of your swab test schedule before check-in • Must complete the 7 days/ 6 nights quarantine or longer • Swab test result and Bureau of Quarantine Certificate before check-out ***If with companion, proof of kinship or that you are living in the same household.
